We are currently partnering with a local primary care clinic to find the right candidate for the following position :

Home Visiting Nurse Practitioner

Responsibilities : Manage the transitional and chronic care needs of homebound patients including :

• Assessment and Evaluation of current condition
• Take past medical history
• Review symptoms
• Conduct general physical examinations
• Review and reconcile medications
• Screen for depression
• Screen for alcohol and drug abuse
• Identify diagnoses to be used in care management and active medical management for purposes of treatment
• Recommend appropriate measures, including medication prescriptions and skilled needs for the patient
• Efficiently communicate findings in your assessment to the office / supervising physician
• Educate patients on topics such as disease process and prevention, medication, and compliance
