Responsibilities RN - Nurse Navigator - Cardiac Aiken Regional Medical Centers, located in Aiken, South Carolina, is a 273-bed acute care facility providing top quality and safe healthcare to the residents of Aiken and surrounding communities since 1917. Visit us online at : Position Description : To serve as a liaison between the cardiologist, patients, and staff to ensure the Center delivers care that will minimize complications and maximize patient outcomes. The Nurse Navigator will assist with tracking the progress of the program, identifying areas that require improvement and facilitating improvement actions. The Nurse Navigator will provide patient education and assist with program marketing strategies. The Nurse Navigator will collaborate with the Heart Attack / Heart Failure Coordinator, Service Line Directors, and other leaders to improve the overall outcomes of the cardiovascular patients. All other duties as assigned. Duties : Demonstrates the ability to communicate and implement the hospital's mission, vision and values as well as departmental goals.Provides education to the cardiac patient population.Coordinates follow up appointments for cardiac patients.Oversees registry abstraction projects for cardiac care (i.e. GWTG, NCDR, STS)Facilitates cardiac and other performance improvement committees, projects, etc. as indicated.Leverages evidence based best practices to improve care and outcomes of cardiovascular patients to reduce adverse outcomes and complications.Facilitates DSC survey activity in collaboration with Quality Director.Other duties as assigned. Qualifications Requirements : Graduate of an accredited or NLN approved RN program. BSN preferred. At least 3 years' experience in area of practice, 5 years preferred. Proven working relationship with physicians. Experience with patient education. Ability to speak in front of a small group. Excellent organizational skills.
