Key Responsibilities
- Perform initial and ongoing assessments to develop and update personalized care plans
- Deliver skilled nursing services and specialized procedures per physician orders
- Complete timely, accurate documentation including OASIS assessments
- Educate patients and families on safety, medications, and health conditions
- Communicate with physicians regarding changes in patient condition
- Supervise and support LPNs and HHAs, including conducting supervisory visits
- Coordinate care with the interdisciplinary team and participate in team conferences
- Ensure compliance with all clinical, regulatory, and documentation standards
Requirements
What We're Looking For
Active RN license in the state of New MexicoAt least 1 year of clinical nursing experience (home health preferred)Proficiency in OASIS documentation and home health regulationsStrong clinical judgment and attention to detailAbility to work independently with reliable transportationCurrent CPR certificationCompassionate communicator and collaborative team playerValid driver's license and auto insuranceWhy Join Us
