Job descriptionOversee and lead a team of case managers to deliver exceptional care. Collaborate with multidisciplinary teams to develop and implement individualized care plans. Monitor compliance with care standards, policies, and regulations. Act as a primary resource and mentor for team members. Manage client cases, ensuring their needs are met with the highest quality of care. Maintain accurate records and documentation. Valid Registered Nurse (RN) license in Hawaii. Minimum of 3 years of case management experience. Leadership or supervisory experience preferred. Strong organizational, communication, and interpersonal skills. Passion for delivering exceptional care and leading a team.
