Makes follow up visits according to patient, family, and facility needs.\nCommunicates significant findings, problems, and changes in health condition, environment, or unsafe facility conditions to the Clinical Supervisor, physician, facility, and / or other personnel involved with patient care.\nRevises the plan of care in response to identified patient care issues and notifies the Clinical Supervisor, physician, facility, and other team members.\n\nWhat is Required?\n\nCurrent, unrestricted state RN licensee\n1+ year experience in a clinical care setting as a nurse\nAble and willing to travel within branch / office coverage area.\nMust have a dependable vehicle, valid driver's license, and current auto insurance in accordance with state laws.\nMust be able and willing to travel 50%\nAssociates Degree or Bachelor's Degree in Nursing is preferred\nPrevious Home Health or Hospice Experience is preferred\nPrevious experience with HomeCare HomeBase is preferred\n\nYou will report to the Clinical Manager or Clinical Supervisor\n\nThis is not a comprehensive list of all job responsibilities and requirements; a full job description will be provided upon request.\n\nWe value the unique skills of veterans and military spouses.
