• Assessment : Performs continuous, systematic data collection, focusing on patients' physiological, psychological, and cognitive status.
• Diagnosis : Analyzes assessment data to determine appropriate nursing diagnoses.
• Outcome Identification : Identifies individualized, appropriate, and expected outcomes for each patient.
• Planning : Develops a prioritized, goal-directed care plan based on diagnoses and expected outcomes.
• Learning Needs : Assesses educational needs of the patient and significant others and provides appropriate teaching.
• Implementation : Delivers consistent, skilled, and knowledgeable care in accordance with the plan of care.
• Documentation : Accurately documents care and patient responses in a timely and concise manner.
