Job Summary and Qualifications

The Physical Therapist (PT) provides high-quality, patient-centered care by managing the complete process of therapeutic care of an assigned group of patients in coordination with the physician and the patient care team. The Physical Therapist completes patient assessments and participates in developing and implementing individualized care plans focusing on goal achievement, patient engagement, and positive patient experience. The Physical Therapist provides or directs a majority of the patient's physical therapy to support patients throughout the continuum of care.

Must be a licensed driver with an automobile that is insured in accordance with state and / or organization requirements and is in good working order h quality care during their recovery at home. We are Medicare certified. Our patients are under the supervision of a physician and our team of professionals develop an individualized plan of care. Our professionals include registered nurses, licensed practical nurses, therapists, and social workers. We provide the care you need in the comfort of your own home to restore your independence.
