Definition
Responsibilities include providing direct nursing care for reentrants housed in Renewal, Inc.'s Community Corrections Centers in accordance with established policy and procedure. This individual will also serve as case manager for Renewal, Inc. reentrants. This individual will conduct triaging of new intakes at the time of their arrival and work collaboratively with Renewal's Inc. contracted medical provider(s), assisting with patient physicals, medication management, and scheduling follow-up appointments, as needed. This person will work closely with case management and counseling staff to ensure reentrants are medication compliant.

To perform this job successfully, an individual must be able to perform each essential duty satisfactorily. The requirements listed below are representative of the knowledge, skill, and / or ability required.
Education and / or Experience
LPN licensure required. Medical and behavioral healthcare experience preferred. Valid PA driver's license and Act 33 and 34 clearance required. Must be organized, detail orientated, and able to work in fast-paced environment. Individual must be able to interact effectively with clientele who are actively impaired
