Job descriptionConduct patient visits in the home care setting Timely Documentation of patient visits in EMR Perform routine, respite, inpatient, admission, and recertification visits Attend IDT meetings, and coordinate plan of care with interdisciplinary team Assists in development, review and revision of the plan of care. Provides nursing care in accordance with physician’s plan of treatment, and individual plan of care. Observes, records and reports any changes in patient's condition to appropriate personnel and / or physician. Provides ongoing instruction and supervision of patient, family or other health team personnel in the plan of treatment. Participates in education programs, quality assessment activities, nurse’s meetings and other staff meetings as required. RN License in the state of TX 1-2 year of RN experience in a clinical setting preferred 1 year hospice experience preferred Active Driver's License
