Provides specialized nursing care and treatments for critically fragile individuals with special medical and physical needs at St. Mary’s Home. Incumbents may be required to work a variety of shifts. All services will be provided under a person-centered model of care and in support of each individual’s support plan (ISP).
RESPONSIBILITIES
- Conduct daily physical assessments of assigned individuals.
- Administer medications, treatments and tube feedings as prescribed by the physician.
- Receive, transcribe and implement physician’s orders using the electronic health record.
- Manage and provide care to tracheostomy-dependent
- Intervene appropriately in the event of emergencies with individuals or staff.
- Document activities related to nursing and medical care in a timely manner and consistent with established formats.
- Safeguard Individual Rights.
- Assist in developing and updating the program plans of individuals when requested.
- Be knowledgeable about and participate in the provision of Active Treatment programs for individuals.
- Assist with maintaining a safe and sanitary environment in compliance with OSHA regulations.
- Participate in staff meetings and staff development programs as directed.
- Communicate with families and staff.
- Accompany individuals on appointments or field trips when requested.
- Direct / supervise the delivery of nursing care in the absence of the Director of Nursing or an RN.
- Be prepared to precept new nurses, if asked.
- Direct and assist the activities of the Direct Support Professionals (DSPs) in the absence of the Program Manager, RN Supervisor, and / or House Supervisor.
- Report insubordinate DSP behavior or any other flagrant disregard of DSP job assignments, duties or care procedures for individuals to the Program or Assistant Program Managers.
- Follow Event reporting procedures for instances of neglect and abuse.
- Attends all in-services posted as mandatory and completes additional fifteen hours of related training every year.
EDUCATION / EXPERIENCE
Must be a graduate of an accredited school of practical nursingMust have a current license in the state of Virginia or proof of application for suchOne year experience working with individuals with special needs is preferred.Knowledge, skill and experience in specialized nursing techniques, including the care of tracheostomy-dependent preferred.Must have a drivers’ license and provide a copy of an acceptable driving recordMust complete training to drive the organizations’ vehicles including a variety of tie-down proceduresMust complete Therapeutic Options and 2-day Person Centered Thinking coursesCPR and Basic First Aid required.REQUIREMENTS
Must be able to sit, stand, walk, bend, and squat for variable periods of time, adhere to the group home’s dress code policy, and always wear a company-issued ID badge.Ability to lift 35 pounds independently and with or without assistive equipmentThe employee is regularly required to talk or hear clearly. Specific vision abilities required by this job include close vision and ability to adjust focus.Use their hands to handle, control, or feel objects, tools, or controls. Use fingers to grasp, move, or assemble very small objects.
