Key Responsibilities :
- Provide high-quality clinical services while ensuring all documentation meets established standards.
- Communicate effectively with customers and colleagues to address inquiries and concerns.
- Collaborate with the treatment team to develop personalized treatment plans.
- Deliver individual, group, and family interventions as outlined in treatment plans.
- Implement approved therapeutic interventions with clients and families.
- Maintain timely and thorough documentation for all client interactions.
- Participate in weekly Treatment Team meetings to coordinate care.
- Connect clients and families with specific services and resources identified in treatment plans.
- Act as a liaison with other professionals and agencies involved with clients.
- Proactively recruit new referrals to maintain a full caseload.
- Provide 24 / 7 on-call support to clients as needed.
- Ensure compliance with state regulations and organizational standards.
- Engage in assigned training and professional development activities.
Qualifications :
A Bachelor's degree in a human services field (e.g., Social Work, Psychology, Counseling) and 1 year of relevant experience working with children, or 3 years of relevant experience.Why Join Health Connect America?
