Sunbelt Staffing is hiring an experienced Home Health RN for a contract role in Columbus, GA. The RN will provide skilled nursing care in patients homes, complete OASIS assessments, and document all visits using Homecare Homebase (HCHB). This position is ideal for an RN who thrives in an independent work setting while ensuring excellent patient outcomes.

Key Responsibilities :

Conduct comprehensive patient assessments, admissions, recerts, and discharges using OASIS

Deliver direct nursing care and patient education in the home environment

Document visits accurately in Homecare Homebase (HCHB)

Collaborate with physicians and interdisciplinary staff for individualized care plans

Manage caseload effectively while maintaining quality and compliance standards

Qualifications :

Active RN license in Georgia

Minimum 2 years of nursing experience, including at least 1 year in home health

Proficiency in OASIS and Homecare Homebase (HCHB) required

Reliable vehicle, drivers license, and auto insurance

Ability to pass background check, drug screen, and onboarding in compliance with the Joint Commission
