We are currently seeking a full-time Care Coordinator in our Wyncote, PA office.

Job duties include, but are not limited to, the following :

• Maintains a current client roster with necessary information
• Updates logbooks and schedule sheets
• Prepares weekly schedule for field staff and provides copies for distribution
• Assists in coordinating services provided to clients
• Controls and monitors schedule changes
• Checks compliance of visits done
• Obtains weekly visit count and report results to the Supervisor
• Assists in relaying messages to field staff, office staff and community liaisons
