What you'll do
The LMSW - Inpatient Case Manager supports the clinical case management and care team to address patients’ psychosocial needs. The LMSW - Inpatient Case Manager maintains current knowledge of basic healthcare economics, trends and reimbursements methodologies and applies knowledge to daily practices. The LMSW - Inpatient Case Manager recognizes situations that require immediate intervention and continues to develop professionally and apply knowledge and skills necessary to provide care appropriate to each patient.
Clinical and Technical Responsibilities :
Provides case management services as appropriate throughout the life span and in various settings
Actively manages LOS for unit’s patients through partnership and collaboration with all members of the healthcare team
Receives referrals from RN case manager and or healthcare provider for support with psychosocial issues
Provide crucial psychosocial support and services to patients and their caregivers in the emergency room setting as identified
Respond to and prioritize high risk cases such as crisis interventions, protective services, guardianship, support agencies
Completes and documents initial and / or comprehensive psychosocial assessments collaborating with RN case manager and care team for management and interventions (substance abuse, guardianship, domestic violence, homeless, etc.)
Assesses patients’ functional status, cognitive ability, support systems, available resources, and barriers to care to identify post-acute care needs
Coordinates patient family conferences, assists with management / support of patient / family issues
Helps family members and patients that are in need of further conversation regarding advance directives
Develops a discharge plan in coordination with patient, their caregivers, and healthcare team members. Reassess the discharge plan throughout patient stay to determine post-acute needs and communicates discharge plan to patient and family as needed
Responsibilities for Relationships, Collaboration and Teamwork :
Collaborates with other health care team members providing patient- centered care
Keeps collaborating RN case manager informed of status, interventions, and plans to progress patient care to discharge
Works collaboratively with care team to coordinate patient discharge for complex cases as defined by facility criteria
Attends and actively participates in MDRs to report progress with discharge plan and / or collaborates with RN case manager clinical needs
Provides community resource information and facilitates or completes referrals as indicated
Identifies and collaborates with post-acute alternate level of care facilities such as long-term care and rehabilitation services. Ensures discharge plan is confirmed and addresses barriers to discharge
Understand and follow regulatory guidelines. Provide regulatory patient notifications when indicated
Financial Responsibilities :
Responsibly apportions resources
Assists with excessively long length of stay patients when discharge options have been exhausted
Coordinates Medication Assistance as indicated
Performs similar or related duties as requested or directed
What we need from you
Education :
Experience :
Minimum 1 year experience in social work, preferred
Minimum 1 year experience in an acute care setting, preferred
Licensure / Certifications :
NYS Licensure as a LMSW must be obtained within 6 months of hire date, required
Case Management Certificate, preferred

Physical Requirements :

Pay Range
Salary is based on a variety of factors, including, but not limited to, qualifications, experience, education, licenses, specialty, training, and fair market evaluation based on industry standards. The above compensation range represents a good faith belief of the compensation range by Bassett Healthcare Network, and its entities and affiliates, at the time of this posting or advertisement.
Pay Range Minimum : USD $69,245.20 / Yr.
Pay Range Maximum :
USD $103,867.80 / Yr.
