Augusta, ME 04330 Name Clear Instructions : Full legal name and NPI# - Processed through SalesForce Presentation Instructions : MG Attestation Form, Demographics Form, CV (in correct format), 2 references (completed checkster forms), complete Job Applicant - including availability, pay rate, phone interview availability Reason for Coverage : Client has 7-10 shifts open monthly - looking to add MDs to locums roster for PRN ongoing Coverage Dates : 7-10 shifts Shift Schedule : 9 : 00a.m to 6 : 00p.m Call Schedule / Responsibilities : No calls Type of Patients / Cases : Level II Trauma in the Emergency Department Patients per Day / Volume : 1.2 to 1.4 per hour Certifications / Qualifications required : Board Certified in Emergency Medicine Board Eligible or Board Certified : Board Certified EMR : SCM Support Staff : Yes Credentialing Timeframe : 8-10 weeks Will license while credentialing : Yes IMLC State : Yes
Vaccination requirements COVID Vaccine & Flu shot will be required state of Maine mandate for all healthcare workers
