As a Hospice Field Case Manager, you will collaborate with healthcare professionals to ensure comprehensive and coordinated care. Your responsibilities will include supervising home health aides and homemakers, communicating with physicians, and documenting patient information.
This is a fee-for-service position with attractive compensation packages.
Degree in nursing from an accredited institution; BSN preferred
At least two years of experience in medical-surgical, community health, or oncology nursing
Proficiency in Microsoft Office applications
New York State RN licensure and valid NYS driver's license required
Proven ability to work effectively in a team environment
Excellent communication, interpersonal, and problem-solving skills
Able to maintain accurate and detailed records
