A healthcare services company throughout New York is currently seeking a Nurse Practitioner (NP) or Physician Assistant (PA) to join their team.
Responsibilities :
The Nurse Practitioner (NP) / Physician Assistant (PA) will :
- Review and make recommendations on consumer assessments for long term care services and supports.
- Maintain current knowledge of regulatory changes pertaining to quality and validity of assessments.
- Assess cases presented and utilizes clinical competencies to review and validate assessment outcomes.
- Work collaboratively to monitor and address clinical quality and gaps in care.
- Provide clinical and educational assistance to support project initiatives.
- Demonstrate communication skills necessary to interpret age-specific responses.
- Participate in performance, improvement, and quality assurance activities and related meetings.
- Provide clinical and educational support to meet program goals.
- Perform other duties as may be assigned by Management.
Qualifications :
2+ years of relevant work experienceMaster's Degree from a Nurse Practitioner or Physician Assistant ProgramNYS Nurse Practitioner (NP) and / or Physician Assistant (PA) licenseBoard Certified Nurse Practitioner (NP) and / or Physician Assistant (PA)Computer savvyDesired Skills :
Board Certified in Internal Medicine, Adult Health, Community Health, and / or Family PracticeBilingual (English and Cantonese, Mandarin, Bengali, Haitian-Creole, Russian, or Spanish)Experience working with the Adult / Geriatric populations.Home Care backgroundExperience working with individuals with Physical and / or Developmental DisabilitiesKnowledge of Long-Term Care services & programs
