Job description

Overview

The CDC Foundation is seeking a Case Investigator to support the Arizona Department of Health Services to support the fight against COVID-19, Sexually Transmitted Infections (STIs), Foodborne / Waterborne illnesses and other conditions of public health significance. Responsibilities will include engaging with people with laboratory-confirmed, probable and suspected diagnoses of conditions of public health significance to discuss sensitive health information, conduct complex interviews, and provide critical education and guidance to interrupt ongoing disease transmission.

Required Qualifications :

  • Bachelor’s degree or higher from an accredited college or university or 3-5 years of relevant experience in lieu of a degree
  • Strong verbal and written communication skills, including active listening, emotional intelligence
  • Ability to interact in an empathetic and non-judgmental manner with culturally diverse populations and persons experiencing a wide range of social conditions
  • Critical thinking and problem-solving skills, and the ability to use sound judgment in responding to client issues and concerns
  • Ability to adapt to changing environments and receive constructive feedback
  • Ability to use discretion, maintain confidentiality, and ethical conduct
  • Ability to work effectively with all levels of staff, establishing and maintaining collaborative professional relationships
  • Strong time management skills (organization, prioritization, multitasking)
  • Ability to work independently and as part of a multidisciplinary team
  • Proficiency with use of computers
  • Proficiency with use of mobile devices (smartphones, mobile apps)
  • Must maintain a valid driver's license in their state of residence, self-report motor vehicle violations / convictions, and provide proof of acceptable insurance coverage with mandatory minimum coverages
  • Must be comfortable with moderate physical effort, including lifting materials and equipment weighing less than 50 pounds
  • Must be able to use a computer, telephone or cell phone daily, and have access to a secure internet connection. As well as spending 50 to 75 percent of the time viewing a computer screen.
  • Must be comfortable working from home while complying with all requirements for client confidentiality, records, and data security.
  • Must be able to travel and occasionally participate in off-site functions.
  • Must be able to respond to public health emergencies, which may require communicating on short notice

Preferred Qualifications :

  • Experience with patient interview, contact tracing / partner notification, medical record review, health education, community outreach, linkage to care, and / or social services
  • Experience with interviews and investigations of individuals diagnosed with or suspected of being at high-risk for COVID-19 and / or sexually transmitted infections (STIs).
  • Experience conducting telephone-based or in-person interviews, data collection and entry
  • Experience in customer service industry
  • Experience in public health
  • Valid driver’s license and car insurance
  • Spanish language fluency
  • Responsibilities :

  • Obtain and review demographic, identifying, laboratory, clinical and risk information available in surveillance systems in order to determine investigation priorities. Initiate prompt communication with individuals diagnosed with COVID-19, STIs, and other conditions of public health significance through text, phone calls, email and other communication platforms as necessary.
  • Employ dynamic communication and interpersonal skills, cultural competency / sensitivity, tactful language and empathetic interviewing skills to build rapport and maintain trust with patients of varied backgrounds.
  • Verify case or contact’s identity during initial communications and prior to disclosing confidential information. Assure confidentiality and carry out efforts to locate and communicate with patients in a manner that preserves the confidentiality and privacy of all involved.
  • Obtain and accurately document detailed information regarding case diagnosis, demographics, symptoms and their onset date(s), clinical course, underlying medical conditions, and other risk factors.
  • Review clinically documented and self-reported symptom history to determine the contact elicitation window.
  • Provide approved information on self-isolation procedures, including what to do if symptoms worsen or new symptoms develop.
  • Assess case’s ability to safely and effectively self-isolate at home, with adequate water, food and other necessities such as bathrooms. Identify barriers to necessary interventions and facilitate appropriate referrals, per local protocols.
  • Maintain open communication and work closely with a team of public health professionals. Communicate with the team lead and site lead when communication attempts are unsuccessful, problem solving alternate methods for outreach. Elevate complex situations to the site lead for further guidance.
  • Participate in requisite trainings, regular program meetings and quality monitoring improvement activities in order to ensure and enhance the quality of case investigation activities and program outcomes.
  • Contribute to development of protocols, trainings, work flows as directed by the health department
  • Special Notes :

    This position requires the operation of a motor vehicle as a component of the job responsibilities and duties. In adherence with CDC Foundation’s Driver Safety Motor Vehicle Policy (“Policy”), offers for employment for this position require the Applicant to provide authorization to the CDC Foundation to conduct a Motor Vehicle Record (“MVR”) review both pre-employment and also annually post-employment while in this position. Additionally, if an offer of employment is accepted, employees in these positions must possess and maintain a valid driver's license in their state of residence, self-report motor vehicle violations / convictions, and provide proof of acceptable insurance coverage with mandatory minimum coverages as set forth within the Driver Safety Motor Vehicle policy. All offers of employment and continued employment for this position are contingent upon meeting all minimum qualifications of this Policy.

    This role is involved in a dynamic public health program. As such, roles and responsibilities are subject to change as situations evolve. Roles and responsibilities listed above may be expanded upon or updated to match priorities and needs, once written approval is received by both the CDC Foundation and Arizona Department of Health Services, in order to best support the State of Arizona in their public health programming.

    This position is Hybrid and located in Arizona. CDC Foundation retains the right to make adjustments to this as required.
