Social Service Designee

A DAY IN THE LIFE OF A SOCIAL SERVICES DESIGNEE

As a social service designee you arrive each day ready to provide exceptional care to our residents. You provide our residents with information regarding services and resources available to help them cope with their illnesses. You work with mental health practitioners to coordinate mental health services that help us meet the mental and physical needs of our residents and help them live their best each day.

Collaborating with physicians, families, and other staff you develop and manage the plan of care for the mental health of each resident. You monitor resident behavior and assist in staff training. Following physician orders and notifying them of changes in a timely manner, you provide the best care for those we serve.

Thanks to your effective planning, your daily quality assurance measures, and the meaningful relationships you establish with our residents our assisted living program continues to achieve excellent care outcomes. QUALIFICATIONS

• Licensed Social Worker (LSW) through Minnesota Board of Social Work, preferred but not required
• Mental health experience preferred
• Long-term care experience preferred
