What You’ll Do

As a Care Coordinator, you’ll play a critical role in ensuring seamless communication, flawless organization, and consistent high-quality care for our clients and caregivers.

Your key responsibilities include :

Client & Caregiver Support

  • Serve as a trusted, professional point of contact for caregivers, clients, and families
  • Personally welcome new clients with a care call and home visit
  • Handle concerns with poise, professionalism, and empathy
  • Maintain proactive communication through regular service check-ins

Scheduling with Purpose

  • Manage and coordinate schedules with accuracy and foresight
  • Balance client needs and caregiver capacity to deliver exceptional care

    • Training & Onboarding

  • Lead warm, professional onboarding experiences for new caregivers
  • Organize and track coaching and training opportunities within care teams

    • Field Visits & Quality Checks

  • Conduct in-home visits, care assessments, and quality assurance checks across Northern Michigan
  • Ensure care delivery aligns with client expectations and organizational standards

    • What We’re Looking For

  • Proven background in office administration (Healthcare, senior care, medical office, or service coordination experience highly valued)
  • Exceptional communicator—professional, empathetic, and clear in all interactions
  • Highly organized and detail-oriented—able to manage multiple priorities with precision
  • Strong problem-solving and leadership skills—proactive in identifying needs and creating solutions
  • Proficiency with Microsoft 365 (Outlook, Teams, Word, Excel) and adaptability with new software
  • Valid driver’s license, insured vehicle, and ability to pass a background check
  • Bachelor’s degree preferred, or equivalent professional experience
