Unit Description – Cardiac Stepdown Unit (CSU / PCU)
Type of Care :
Progressive care unit that provides intermediate-level cardiac monitoring and treatment for patients transitioning from intensive care or requiring higher observation than a general telemetry floor.Focused on post-cardiac surgical and interventional patients , complex arrhythmia management, and cardiac medication stabilization.Supports recovery following MI, cardiac catheterization, valve procedures, CABG, and TAVR .Patient Population :
Adults and geriatric patients with cardiac, respiratory, and hemodynamic instability requiring continuous monitoring.Average Census :
Variable; manages post-cardiac and medical stepdown overflow.Shift & Schedule :
12-hour shifts with rotating weekends and holidays.Float required to similar PCU or telemetry units based on staffing needs.Call may be required as per department scheduling.Primary Responsibilities :
Provide patient-centered care using advanced cardiac and telemetry monitoring systems.Interpret and manage dysrhythmias, respond to alarms, and assist in emergent code situations.Care for patients pre / post cardiac cath, TAVR, pacemaker, or sheath removal .Administer and monitor cardiac drips (non-titrating) and anticoagulation protocols.Perform IV therapy including blood product administration, central / peripheral line management, and arterial line monitoring.Assess and manage patients with respiratory compromise, utilizing CPAP / BiPAP, HFNC, or supplemental oxygen .Collaborate with interdisciplinary teams (hospitalists, respiratory therapy, physical therapy, pharmacy, and social services) to ensure optimal outcomes.Document care and assessments accurately in Epic EMR using SBAR for communication.Skills & Procedures :
Cardiac Monitoring : Dysrhythmia interpretation and management, telemetry troubleshooting.Cardiac Interventions : Pre / post MI care, cardiac surgery recovery, sheath and TR band management.IV / Line Management : Central line, peripheral IV, blood transfusions, arterial line monitoring.Respiratory Support : Non-invasive ventilation, high-flow nasal cannula, and tracheostomy care.Emergency Response : Rapid response / code team activation and participation.Neurological Monitoring : Post-stroke, seizure, and post-TPA care.Wound & Isolation Care : VAC management and infection prevention protocols.Certifications Required :
BLSACLSNIHSS (preferred or required based on facility policy)Experience Requirements :
Minimum 1 year of recent Progressive Care or Cardiac Stepdown experience required.Must have telemetry proficiency and strong cardiac background.First-time travelers accepted.Systems & Equipment :
