Bilingual RN Case Manager

A company is looking for a Bilingual (Spanish / English) Transitions of Care RN Case Manager.

Key Responsibilities

Engage with newly discharged patients to assess needs and develop care plans

Coordinate care with healthcare providers and community resources for post-discharge support

Educate patients on chronic conditions and ensure adherence to care plans

Required Qualifications

3-4 years of RN Care Management or telephonic case management experience

An unrestricted Compact RN license obtained in the United States

Experience in a performance and productivity metrics-driven environment

Health insurance experience, particularly in Medicare Advantage

Experience in Transitions of Care case / care management
