LCSW - Hospice Licensed Clinical Social Worker, FT

Licensed Clinical Social Worker - Join Our Compassionate Hospice Care Team Pacific Hospice & Palliative Care is an ACHC accredited hospice agency offering high standards of quality, safety, and organizational performance. We are seeking an experienced, dedicated and empathetic fulltime Licensed Clinical Social Worker (LCSW) to join our interdisciplinary team. This is a meaningful opportunity to provide emotional support, guidance, and advocacy for patients and families during end-of-life care.

Locations : San Antonio, TX headquarters, in-home and facility visits to Hospice patients in the greater San Antonio area

Schedule : Monday-Friday, full-time with occasional visits may occur outside of regular schedule and will be paid at PRN rates.

Your Role :

As an integral member of our hospice team, you will :

• Provide psychosocial assessments, counseling, and support to patients and families
• Help families navigate end-of-life planning, resources, and community services
• Participate in weekly interdisciplinary team meetings to coordinate high-quality care
• Advocate for patient rights, wishes, and care goals
• Offer crisis intervention and bereavement support
• Maintain accurate, timely documentation in accordance with regulatory standards
• Collaborate with nurses, physicians, chaplains, and other team members to deliver holistic care
