Position : Speech Therapist
Department : HOME HEALTH
Job Summary
Maintains minimum requirements of a master’s degree in Speech-Language Pathology, current MS SLP license, and current Certificate of Clinical Competence or CFY. Provides direct and indirect patient care for adult and pediatric patients in a variety of settings to include performing evaluations, developing treatment plans, providing appropriate treatment, and planning discharge per standards set by the American Speech-Language Hearing Association. Communicates with physicians, nurses, dieticians, social workers, co-workers, and caretakers regarding patient assessment, recommendations, plan and prognosis.
Essential Duties and Responsibilities
- Professional Responsibilities :
- Appropriately administers and interprets standardized assessments for articulation, language, and cognitive skills
- Establish appropriate diet recommendations and compensatory strategies based on clinical dysphagia assessment
- Establishes appropriate and pertinent plan of care based on chart review, diagnosis, and clinical findings
- Accurately and efficiently completes E-Care documentation
- Meets CEU requirements for assigned intervals to maintain current, valid MS State Dept of Health license and ASHA CCC or CFY
- Actively participates in the department’s performance improvement (PI) activities
- Adheres to and utilizes appropriate infection control policies
- Participates in orientation of new personnel
- Attends mandatory staff meetings and remains current with HealthStream requirements
- Meets annual requirements as defined by the facility
