Rate : Up to $95,000 per year

Shift : Full-Time

The Gardens at West Shore, a leading skilled nursing facility, is seeking a dedicated Registered Dietitian to join our team. This role involves implementing, coordinating, and evaluating medical nutrition therapy, providing education to residents and families, and supporting the planning and execution of the facility's food and nutritional services.

Key Responsibilities

• Conduct preliminary and comprehensive dietary assessments for residents.
• Develop individualized dietary care plans, identifying specific goals and resident needs.
• Engage residents and families in creating and reviewing care plans.
• Ensure dietary staff understand and utilize care plans in daily service delivery.
• Identify and recommend appropriate medical nutrition therapy based on patient charts.
• Ensure care plans include necessary special equipment or utensils for residents.
