What We're Looking For : Education : - Graduate of an accredited school of nursing required. - 4 Year / Bachelors Degree, preferred. - If your RN nursing license has been inactive, retired or lapsed for five years or more and you have not been licensed in another state during the last five years, successful completion of a Board approved refresher course is required prior to reactivating or reinstating your license. In order to be considered for current licensure status, you must apply for reinstatement within one year of completing the refresher course. Experience : One year relevant experience, preferred. Licensure / certification / registration : Current RN licensure in appropriate state, required. What You'll Do : Demonstrates strong assessment skills to uncover the patient’s story, integrating subjective and objective data, physical and psychosocial factors, personal preferences, and learning needs. Utilizes ongoing data interpretation to recognize patterns and determine both immediate and future care needs. Collaborates with patients and their support systems to identify priorities and incorporate preferences into individualized care plans. Engages the interdisciplinary team, identifies resources, sets realistic goals, and adjusts plans as patient needs evolve. Acts as a patient advocate while implementing the interdisciplinary care plan. Collaborates with the healthcare team to educate patients, prioritize interventions, and ensure timely completion of nursing and medical treatments. Administers and evaluates medication therapies, provides routine nursing care (IVs, dressing changes, etc.), and responds effectively in critical situations such as CPR. Safely transfers and transports patients and handles necessary equipment as required. Continuously assesses patient progress toward goals and outcomes, seeking input from patients and their support systems. Modifies the plan of care in collaboration with the interdisciplinary team to achieve desired results. Communicates the individualized plan of care and patient progress to the entire care team, including the patient and their support system. Utilizes clear communication tools such as SBARQ and ensures timely documentation. Delegates tasks appropriately to ensure safe, effective care while considering task complexity, staff competence, and patient response. Holds self and others accountable for delivering quality, patient-centered care. Demonstrates independence and professionalism in nursing practice according to state regulations, ANA standards, and the Nurse Practice Act. Engages in shared governance, process improvement, and ongoing professional development to enhance practice. Builds and maintains positive relationships with team members and internal customers.
