Summary :
The Staff LPN at Medical Center delivers primary care medical services to qualified patients in accordance with Company Standards. This role includes recording patient medical history, administering prescribed medications, and coordinating patient care through case management, disease management, ER / CC utilization, and triage.
Responsibilities :
- Administer prescribed medications and treatments to patients.
- Accurately document patient medical history and information in the electronic medical records system.
- Coordinate patient care through case management and disease management.
- Assist in triage, emergency room, and critical care utilization.
- Comply with all medical center policies and procedures to ensure high-quality care.
- Follow the company’s drug-free and tobacco / nicotine-free policy
- All other duties as assigned.
Experience / Qualifications :
Proven experience as an LPN in a clinical setting, preferably in primary care.Bilingual in Spanish or CreoleStrong communication and interpersonal skills.Excellent attention to detail and organizational abilities.Ability to work independently and collaboratively in a fast-paced environment.Proficiency in using computer systems for medical record-keeping.Must be able to legally work in the USA.Education and Certification(s) / License(s) :
