Salary : $95,000 annually + $5,000 retention bonus
Productivity : 28 visits per week
Schedule : AM / PM / Evenings (after-school patients required)
We are seeking a Full-Time Speech Language Pathologist (SLP) to join our home health team. This role is ideal for clinicians who are passionate about serving both pediatric and adult populations, value flexibility, and are committed to delivering high-quality, patient-centered care.
Position Summary
The Speech Language Pathologist is responsible for the assessment, diagnosis, treatment, and prevention of speech, language, cognitive-communication, and swallowing disorders. The SLP provides services in accordance with physician orders and agency standards and supports both clients and caregivers to improve functional communication and quality of life.
What You’ll Do
- Provide clinical oversight and guidance to Home Health Speech Language Pathology Assistants (SLPAs)
- Deliver speech therapy services according to physician orders
- Assess and evaluate patients’ therapeutic, rehabilitative, and functional status
- Develop and implement individualized Plans of Care (POC) consistent with evaluations
- Participate in the development of each client’s comprehensive treatment plan
- Identify, develop, or adapt activities that enhance daily functioning and quality of life
- Assess the home environment for safety concerns and recommend appropriate interventions
- Monitor and report patient progress or changes in condition to the Director of Nursing and physician
- Coordinate care with other healthcare providers and community settings (schools, daycare centers, workplaces, adult day care, residential facilities)
- Educate family members, caregivers, and other personnel on treatment goals and interventions
- Assist in identifying additional services needed by clients and families
- Complete and submit clinical documentation and progress summaries in a timely manner
- Participate in discharge planning as appropriate
- Ensure compliance with all local, state, and federal regulations
