Position Overview :
We are currently seeking a highly Experienced Acute Care Inpatient Case Manager with strong discharge planning expertise to join our team. This role requires professionals who can step in with a minimal training, adapt quickly, and manage patients efficiently in a fast-paced Acute Care Environment.
Key Responsibilities :
- Perform comprehensive case management for acute inpatient populations.
- Lead and coordinate discharge planning to ensure smooth transition of care.
- Collaborate with physician, nursing, and interdisciplinary teams to optimize patient outcomes.
- Utilize EPIC EMR for documentation and care coordination.
- Deliver efficient patient-centered care while managing complex cases.
