Per Diem Nurse Practitioner / Physician Assistant
Location: Orange County, FL and surrounding areas.
About the Role We are seeking a compassionate and driven Nurse Practitioner or Physician Assistant to perform in-person annual preventive health assessments for Medicare, Medicaid, and ACA members. This role provides a unique opportunity to deliver care directly within members' homes-offering a no-cost service that enhances health outcomes and fosters patient trust.
Schedule & Compensation - Flexible scheduling: Days and evenings available
- 60-75 minutes per appointment
- Market-rate compensation per completed assessment
- Mileage reimbursement after the first 30 miles daily, based on current IRS rate
Responsibilities - Conduct high-quality in-home health risk assessments
- Document and submit chart notes within 24 hours
- Respond to charting queries within 48 hours
- Offer a minimum of 25-30 appointment slots monthly
- Maintain consistent communication with clinical leadership
- Participate in training sessions and team meetings
- Navigate member homes with sensitivity and professionalism
- Safely lift up to 20 lbs and perform kneeling or bending when necessary
Education & Experience - NP: Master's or Doctorate from accredited Family (FNP), Adult-Gerontology (AGNP), or Adult (ANP) program
- PA: Master's Degree from accredited program
- Preferred: Home health experience and 1+ year of in-home assessments
Certifications - Active NP or PA license in practicing state
- Board certification: ANCC, AANP, or NCCPA
- Medicaid ID and Medicare PTAN number
- Current BLS certification
- Valid driver's license
- Proof of auto insurance compliant with local requirements
Skills & Competencies - Proficient with EMR systems and clinical documentation
- Familiarity with CMS and ACA risk adjustment models
- Strong verbal and written communication skills
