Job Title : Registered Nurse (RN) – Outpatient Primary Care

Job Location :

  • Primary : 2400 Moorpark Avenue, Suite 300 – San Jose, CA
  • Secondary : 266 N. Jackson Avenue #1 & #4 – San Jose, CA

    • Salary Range : $52.72 – $64.30 per hour

    Position Summary :

    We are seeking a dedicated Registered Nurse (RN) to support providers and serve as a clinical lead within a primary care team. This role involves delivering integrated nursing services in an outpatient setting, including triage, vaccine administration, health education, and coordination of care.

    • Key Responsibilities :

  • Triage incoming calls and walk-ins
  • Review lab results and collaborate with providers on abnormal findings
  • Communicate across departments to ensure integrated care
  • Train and oversee nursing students
  • Administer vaccines (Pediatrics and Adults), perform POC tests, ear lavage, wound care, assessments, and venipunctures
  • Provide onboarding services including TB testing and vaccinations
  • Participate in off-site events and oversee ancillary staff
  • Transport vaccines and monitor temperature compliance
  • Assist in developing training materials and conducting staff competency checks
  • Create patient education handouts and documentation guides
  • Perform other related duties and projects as assigned

    • Qualifications :

  • BSN preferred or equivalent education / training
  • Minimum 2 years of nursing experience
  • Active California RN License
  • CPR Certification required; ACLS and PALS preferred

    • Skills & Competencies :

  • Proficiency in MS Office and Electronic Health Records
  • Cultural sensitivity and ability to bridge cultural differences
  • Strong communication and teamwork skills
  • Ability to work effectively in a multidisciplinary team

    • Working Conditions :

  • Office-based clinic environment with standard safety protocols
  • Occasional evening and weekend work required

    • Physical Requirements :

  • Ability to sit for extended periods, operate a computer, and perform manual tasks
  • Occasional lifting, pushing, pulling up to 25 lbs
  • Mental demands include comprehension, decision-making, and resilience
