A&I Avenues is growing, and we're looking for a dedicated and compassionate LTSS Ongoing Case Manager to join our team! In this vital role, you'll support individuals with disabilities and long-term care needs by ensuring they receive high-quality services and access to the resources they need to thrive.

Key Responsibilities

As a Case Manager, you'll be the key advocate and connector for your clients, guiding them through complex systems, building customized Service Plans, and coordinating care that truly meets their individual needs. Your responsibilities will include :

  • Building personalized Service Plans that reflect your clients' goals, strengths, and needs.
  • Conducting eligibility assessments during enrollment and annual reviews.
  • Monitoring service delivery to ensure quality, consistency, and compliance.
  • Empowering clients and families with information, education, and access to resources.
  • Coordinating care with providers, guardians, and other support systems.
  • Resolving concerns and navigating appeals to uphold client rights and satisfaction.
  • Maintaining accurate documentation to ensure ongoing funding and regulatory compliance.

  • Required Qualifications :

    Preferred : Bachelor's degree in human services or related field.

    OR : 5 years of relevant experience in long-term care / disability services.

    OR : A combination of education and experience that prepares you for the role.

    Additional Requirements :

  • Valid driver's license and reliable personal vehicle with insurance.
  • Personal mobile device for job-related communication.
  • Comfort working in a fast-paced, evolving environment.
  • Bonus if You Have :

  • MSW or experience supervising interns
  • Experience navigating Medicaid waivers or long-term care systems
  • Work Environment & Expectations

    This role offers a hybrid work model that balances remote work with regular in-person visits to clients, ensuring hands-on support and meaningful engagement in the community. Case Managers are based in, or willing to relocate to, the Colorado Front Range area to effectively serve clients in Broomfield, Boulder and Gilpin Counties.

    The position is structured around a standard 40-hour work week, with flexibility built in to accommodate client needs and scheduling demands. Any overtime must be pre-approved to maintain work-life balance and operational efficiency.

    This dynamic environment requires adaptability, strong time management, and a commitment to providing high-quality, person-centered care both in the field and virtually.
