Would you like to join a rapidly growing, locally owned hospice with a true commitment to quality patient care? James River Hospice is seeking a compassionate and dedicated Hospice Baylor RN to support our patients and families in Richmond, VA.

As a Baylor nurse, you will provide case management for hospice patients on an intermittent basis in their homes while working closely with the interdisciplinary team to ensure exceptional, patient-centered care.

Schedule : Saturday and Sunday (9 : 00 AM 5 : 00 PM)

Responsibilities :

• Complete initial patient and family assessments to identify hospice needs
• Develop individualized care plans and update them as needed
• Coordinate with the interdisciplinary team and supervise LPNs and Hospice Aides
• Provide direct patient care, pain management, and emotional support
• Monitor and document symptoms and changes in condition
• Communicate and obtain physician orders as appropriate
• Participate in on-call rotation per policy
• Other duties as assigned
