Position Summary :
The Case Manager is responsible for coordinating and managing comprehensive services for individuals and families to ensure they receive appropriate care and support. This role involves conducting assessments, developing individualized care plans in collaboration with the Interdisciplinary Team (IDT), facilitating access to services, leading safe discharge planning, and arranging for home health and durable medical equipment (DME) as needed. The Case Manager monitors client progress to promote well-being, independence, and optimal outcomes.
Key Responsibilities :
- Conduct thorough assessments to identify client needs, strengths, and goals.
- Develop and implement individualized care or service plans in collaboration with the IDT, clients, and relevant stakeholders.
- Coordinate and refer clients to appropriate services, including healthcare, housing, employment, education, and social support systems.
- Lead safe discharge planning, including timely requests for home health services and appropriate DME.
- Monitor client progress and revise care plans as necessary to ensure effective outcomes.
- Maintain accurate, timely, and compliant documentation in accordance with organizational and regulatory standards.
- Schedule and coordinate appointments and transportation services.
- Manage skilled service authorizations by reviewing medical necessity and communicating with health plan or medical group case managers.
- Identify benefit exclusions based on health plan contracts.
- Advocate for clients to ensure access to essential resources and services.
- Collaborate with multidisciplinary teams, community partners, and service providers for safe placements / discharge planning.
- Provide crisis intervention and support when needed.
- Ensure adherence to all applicable laws, regulations, and organizational policies.
- Participate in team meetings, training sessions, and ongoing professional development.
- Perform other related duties as assigned.
Qualifications :
- Required a licensed nurse (LVN) with a minimum of 2 years of case management experience in a skilled nursing facility, RN Preferred.
- Relevant certification or licensure in case management is a plus.
- At least 2 years of experience as a charge nurse in a skilled nursing facility.
- Strong knowledge of community resources, healthcare systems, and social services.
- Excellent communication, organizational, and critical thinking skills.
- Ability to work independently and collaboratively within a team environment.
