Job Title : Registered Nurse

Experience : Minimum 1 full year of Adult Home Health Experience or 1 full year of Hospice experience. Recent specialty experience required.

License : Must have a valid nursing license.

Certifications : CPR and BLS required.

Must-Have : Proficiency in OASIS and HomeCare HomeBase (HCHB) systems, used within the last 6 months, or willingness to complete a retraining course. Reliable transportation, a current driver's license, and agency-required liability insurance. Willingness to travel up to 75 miles from the home clinic.

Job Description :

• Provide, coordinate, and direct home nursing care according to physicians' orders based on agency policies and procedures.
• Supervise and evaluate Home Health Aides and Licensed Practical Nurses / Licensed Vocational Nurses.
• Conduct home visits and attend periodic office meetings.
• Average 5-8 patients per day, with productivity expectations of 30 points per week.
• Types of cases include Start of Care, Wound Care, Follow-up Visits, etc.
• Complete all documentation within 24 hours of the patient appointment.
• Be open to traveling from patient home to patient home, excluding commute to and from the first and last patient.
• Perform duties efficiently to meet the weekly productivity targets.
• Float to another nearby clinic if necessary to pick up additional patients.
