Job descriptionConduct home health visits to patients' homes within the territory and the surrounding area. Perform comprehensive assessments of patients' health status, including physical, emotional, and environmental factors influencing their well-being. Develop and execute personalized care plans tailored to meet the unique needs and goals of each patient, in collaboration with the healthcare team and under physician guidance. Administer skilled nursing care, encompassing wound management, medication administration, IV therapy, and patient education on disease management and preventative care measures. Maintain accurate and timely documentation of all patient interactions, assessments, interventions, and outcomes utilizing Oasis and Homecare Homebase software platforms. Foster interdisciplinary collaboration by actively participating in team meetings, sharing insights, and coordinating care transitions to ensure continuity and effectiveness of care delivery. Establish and maintain open lines of communication with patients, families, and caregivers, providing support, education, and advocacy to enhance patient understanding and engagement in their healthcare journey. Active PennsylvaniaRegistered Nurse (RN) License. Active American Heart Association (AHA) or American Red Cross Basic Life Support (BLS) certification. Minimum of 1 year of experience in home health nursing. Minimum of 1 year of experience with Oasis and Homecare Homebase software. Excellent clinical assessment and critical thinking skills. Strong communication and interpersonal abilities. Reliable transportation and willingness to travel within the assigned territory.

Sunbelt Staffing is seeking an experienced and compassionate Registered Nurse (RN) for a 13-week Home Health contract in Lewistown, PA. This position offers the opportunity to provide personalized care to patients in the comfort of their homes.

- Location : Lewistown,PA, and surrounding areas
- Company : Sunbelt Staffing, on behalf of a Nationwide Home Health Agency
- Position Type : Contract, 13-week term with possibility of extension upon successful completion
- Schedule : Mon-Fri, 8 : 00 AM - 5 : 00 PM with occasional weekends and on-call
- Compensation : Competitive salary, mileage reimbursement, medical, dental, vision, and 401 (k) benefits after 90 days
- Mileage : .48c visit to visit, 200 miles per week average
- Productivity : 25units per week, 30-32 visits
