Admitting Nurse; Registered Nurse

These values - integrity, patient-centered, respect, accountability, and compassion - must guide what we do, as individuals and professionals, every day.\n\nThe Admitting Nurse is primarily responsible for admission and resumption of care visits of patients for home care services, development of the patient plan of care, coordination with the patient's provider and other disciplines for the provision of services, and safe delivery of patient. The Admitting Nurse must have a good understanding of OASIS and must have good assessment skills that enable them to recognize the need for other disciplines.\n\nEEO / AA / Disability / Veteran\n\nResponsibilities\n

• 1. Completes an initial assessment of the patient and family to determine home care needs. Provides a complete physical assessment and history of current and previous illnesses.\n
• 2. Re-evaluates on resumptions of care and regular intervals as identified in Agency policies and procedures.\n
• 3. Develops a care plan which establishes goals, based on nursing diagnosis, and incorporates therapeutic, preventive, and rehabilitative nursing actions. Includes the patient and the family in the planning process.\n
• 4. Collaborates with providers, other disciplines, and communicates all changes promptly.\n
• 5. Assesses payment sources and is responsible for ensuring ongoing authorizations as necessary.\n
• 6. Educates and instructs patients / caregivers in their ongoing medical maintenance. Begins discharge planning on admission.\n
• 7. Completes all documentation within twenty-four (24) hours. Must include changes in condition, follow-up with the provider or other disciplines, with supporting documentation.\n
• 8. Evaluation of patient progress and prompt action.\n
• 9. Oversight of LPNs and Home Health Aides.\n
• 10. Participates in case management conferences and documents per guidelines.\n
• 11. Schedules patients independently according to the individual plan of care.\n
• 12. Attends all staff meetings, in-services, and other required meetings. Compliance with all mandatory requirements.\n
• 13. Follows policies and procedures.\n
• 14. Ensures the arrangements for equipment and other necessary items and services are available.\n
• 15. Thorough and accurate OASIS documentation and revisions.\n
• 16. Case management of a modified caseload as needed.\n
• 17. May perform other duties as assigned.\n\nQualifications\n\nEDUCATION\n\nNursing diploma or Associate's degree in Nursing required. Bachelor's in Nursing preferred.\n\nEXPERIENCE\n\nMinimum of two (2) years of nursing experience preferred. Previous home care experience preferred, and OASIS experience required.\n\nLICENSURE\n\nRegistered Nurse with a valid (or eligible) Connecticut license required. A valid driver's license and proof of car insurance are required when using a personal vehicle for company business. Access to a dependable vehicle in order to travel to multiple stops a day. CPR certification is required and must be maintained. Candidates who do not have certification are required to obtain it within 3 months (90 days) of hire.\n\nSPECIAL SKILLS\n\nUnderstanding of Medicare certification and OASIS documentation experience. Strong organizational skills and the ability to work independently. Strong computer skills and the ability to learn new software. Ability to manage a variety of assignments while establishing priorities and ensuring that necessary duties are completed.\n\nPHYSICAL DEMAND\n\nAbility to perform tasks involving physical activity, which includes heavy lifting, extensive bending, administering CPR, patient bracing, standing, and stairs.\n\nYNHHS Requisition ID\n\n159461
