High Point Residence – Salem South

Supportive Living Community; 6am-6pm; Sunday-Thursday

We are seeking a compassionate and skilled Registered Nurse (RN) or Licensed Practical Nurse (LPN) to join our team in a full-time, daytime position at our memory care community. This role is essential in delivering high-quality nursing care aligned with each resident’s individual assessment and care plan, under the guidance of the Wellness Director.

Position Responsibilities :

Deliver routine daily nursing care and services to residents.

Perform vital sign checks and health assessments.

Coordinate with CNAs to collect information on resident conditions and needs.

Administer medications and ensure accurate documentation.

Provide clinical supervision and leadership to care staff.

Support a positive, resident-focused environment.

Other duties as assigned by the Executive Director

Qualifications :

Current Illinois RN or LPN license in good standing.

Recent experience in a Memory Care community

Strong clinical, assessment, and supervisory skills.

Familiarity with medication administration and care documentation.

Requirements :

High school diploma or GED.

Completion of a state-approved nursing assistant program.

Active status on the IDPH Health Care Worker Registry.

Ability to lift up to 25 lbs and perform physical tasks typical of CNA support.

Valid CPR certification (recently expired accepted; recertification available).
