Job Title : Visit Nurse – Hospice RN (Growth Opportunity)

Compensation : $40 to $46 per visit.

Benefits : Full Benefits Package (when transitioned to full-time)

Location : Greater Twin Cities Metro Area – 21-county service area

Schedule : Part-Time to Start | Potential to Transition to Full-Time as Patient Volume Grows

Since launching in 2023, our CHAP-accredited team has delivered compassionate, dignified in-home hospice care across the Twin Cities, tailoring support to each patient’s physical, emotional, and spiritual needs. We offer a full spectrum of hospice services—including routine care, respite, continuous, and inpatient support—alongside bereavement, integrative therapies, and 24 / 7 emotional support .

Position Summary :

We are seeking a Hospice Visit Nurse (RN) who thrives in the field, values patient relationships, and is excited to grow with a mission-driven organization. This role will begin as part-time, ideal for a nurse seeking flexibility, and transition to full-time as we onboard additional clients. This is a unique opportunity to be part of something new, meaningful, and built to last.

Key Responsibilities :

• Travel to patients’ homes, assisted living, or memory care settings to deliver routine and PRN visits
• Provide symptom management, pain control, wound care, and clinical assessments
• Support patients and families through active listening, education, and compassionate communication
• Collaborate with the interdisciplinary team, including MDs, social workers, chaplains, and intake staff
• Maintain accurate and timely documentation in the EMR system
