Job descriptionProvide case management services to adults with substance use disorders and / or other co-occurring mental illness. Provide case management, psychoeducation, assessment, treatment planning, and backing of the support services under the supervision of the Director of Clinical Services. Coordinate all aspects of care with other members of the interdisciplinary team. Maintain at least an overall score of 90% in all areas of chart review. Participate on committees and in quality improvement activities as assigned. Maintain credentialing or licensure necessary for the position and continually update professional skills through in-service training and other sources. Aid patients in obtaining necessary services, such as financial assistance, housing assistance, food, clothing, medical services, educational services, vocational services, etc. Build and maintain data reports as assigned Build and maintain positive relationships with referral agencies Facilitate psychoeducation groups Teamwork- Accountable to team, works to meet established deliverables, appreciates view of team members, respectful. Ability to work well with individuals of diverse ages, cultures, races, religions, socioeconomic backgrounds, disabilities, gender and sexual orientation. Trauma Informed Care- Practices Trauma Informed Care by creating safe, caring, inclusive environments for all patients. While performing the duties of this job, the employee is regularly required to, stand, sit; talk, hear, and use hands and fingers to operate a computer and telephone keyboard reach

Qualifications
Certified or Licensed by the Ohio Chemical Dependency Professionals Board or the Ohio Counselor, Social Worker, Marriage and Family Therapist Board (CDCA or above). Minimum of 1 year paid work experience preferred. Previous experience with database and / or electronic charting is helpful.
