Job purpose
Provide case management, psychoeducation, assessment, treatment planning, and backing of the support services under the supervision of the Director of Clinical Services. Please note, job duties may be increased, decreased or combined as the needs of the agency dictate.
Duties and responsibilities
General :
Provide case management services to adults with substance use disorders and / or other co-occurring mental illness.Provide case management, psychoeducation, assessment, treatment planning, and backing of the support services under the supervision of the Director of Clinical Services.Coordinate all aspects of care with other members of the interdisciplinary team.Maintain at least an overall score of 90% in all areas of chart review.Participate on committees and in quality improvement activities as assigned.Maintain credentialing or licensure necessary for the position and continually update professional skills through in-service training and other sources.Case Management
Aid patients in obtaining necessary services, such as financial assistance, housing assistance, food, clothing, medical services, educational services, vocational services, etc.Build and maintain data reports as assignedBuild and maintain positive relationships with referral agenciesFacilitate psychoeducation groupsQualifications
Certified or Licensed by the Ohio Chemical Dependency Professionals Board or the Ohio Counselor, Social Worker, Marriage and Family Therapist Board (CDCA or above). Minimum of 1 year paid work experience preferred. Previous experience with database and / or electronic charting is helpful.
