Humana / iCare is looking for a dedicated RN Care Manager to support members through our Family Care Partnership (FCP) program in Racine / Kenosha counties. In this role, you will provide essential case management services to adults, ensuring they receive quality care that is both effective and efficient. Your contributions will directly improve the lives of the members you serve.
The FCP RN Care Manager will collaborate within an Interdisciplinary Team (IDT) to manage member care. This position involves conducting home and community visits to perform nursing assessments and to facilitate access to necessary services.
Key Responsibilities :
- Conduct comprehensive nursing assessments and offer insights to aid in the development of care plans.
- Establish and track wellness and health promotion initiatives based on individual member assessments and outcomes.
- Reassess members' health and wellness periodically, particularly in response to any changes in health status.
- Execute routine health monitoring activities tailored to members' needs.
- Foster trusting relationships with members, their families, and external agency personnel.
- Assess and intervene during crisis situations involving psychiatric, behavioral, or medical issues while collaborating with team members to ensure safety.
- Evaluate members’ abilities to manage their own care, including medication, treatments, and access to healthcare services.
- Provide or arrange necessary services that align with the members’ identified needs and assess the effectiveness of these interventions.
- Assist in the development and refinement of program policies and contribute to quality improvement initiatives.

Required Qualifications :
Active Registered Nurse license in the State of Wisconsin.A minimum of two years of relevant healthcare experience.Must be willing and able to travel within Racine / Kenosha counties.Preferred Qualifications :
Bachelor of Science in Nursing (BSN).Prior experience in home health, managed care, or case management.Experience working with individuals with physical or intellectual / developmental disabilities, or older adults.Certification in Case Management.Foundational knowledge of Medicaid and Medicare benefits.Additional Information :
Work Environment : This role is primarily field-based, with responsibilities carried out at member locations or partner facilities.Travel : Up to 50% travel within Racine / Kenosha counties.Schedule : Monday - Friday, 8 : 30 am - 5 : 00 pm Central Standard Time (CST).Internet and Home Office Requirements :
Employees must have a minimum Internet download speed of 25 Mbps and upload speed of 10 Mbps; a wired or DSL connection is recommended.Satellite or cellular connections are permissible with prior approval.Driving Requirements :
As part of Humana's driver safety program, this role requires a valid state driver's license, as well as proof of personal vehicle insurance with specified coverage limits.
Tuberculosis Screening :
This position is patient-facing and requires participation in Humana's TB screening program.
