Job Description
Quality evaluation and analysis of data relating to nursing and personal care services and related documentation in order to meet total nursing needs of residents. Assists in planning, organizing and directing nursing services.Presents admission and quarterly care plans to residents and families. Acts as liaison between Administrator and Quality Assessment and Assurance Committee.
Skilled Nursing Facility / Part of Continuing Care Retirement Community
Partners with a second full time Care Plan Coordinatorin191bed / Non-Medicare / Medicarefacility
Rotates on call with all administrative nursing.
Weekly wound assessments as needed.
Qualifications
Minimum of 2 years experience in Long Term Care setting, preferred.
Must have current NC RN license
Must have working knowledge of regulations of licensure required by state of NC
Must have ability to work well with facility staff, analyze data, and identify problems and propose solutions
