THIS ROLE IS REMOTE HOWEVER TO BE CONSIDERED FOR THIS ROLE YOU MUST RESIDE IN OHIO in either Clinton, Clermont, or Hamilton County.

Monday through Friday
8 : 00 AM - 5 : 00 PM EST
Pay Rate is $35.75

Healthcare Consultant is responsible for telephonically and / or face to face assessing, planning, implementing and coordinating all case management activities with members to evaluate the medical needs of the member to facilitate the member’s overall wellness. Develops a proactive course of action to address issues presented to enhance the short and long-term outcomes as well as opportunities to enhance a member’s overall wellness through integration. Services strategies policies and programs are comprised of network management and clinical coverage policies.

Requirements

Bachelor's degree in Social Work

Active and in good standing LSW for Ohio

3 years of behavioral health or social work experience

1 year of case management experience

Computer Saavy skills (Microsoft Office : Word, Excel, and Outlook)

Must be willing and able to travel up to 50-75% of the time in Clinton, Hamilton, or Clermont County. Reliable transportation required. Mileage is reimbursed.
