The Community Resource Navigator is a healthcare worker who is a trusted member of and / or has an unusually close understanding of the community served. This role supports patient care in accordance with established methods, techniques, and standards of The Corvallis Clinic.

Schedule : Monday through Friday 8 am to 5 pm.PST

Location : 3680 NW Samaritan Drive, Second Floor, Corvallis, OR 97330

Primary Responsibilities :

• Will participate and maintain a culture within The Corvallis Clinic consistent with the content outlined in the Service and Behavioral Standards Caregiver document. To this end, employee will be expected to read, have familiarity with, and embrace the principles contained within
• Care Coordination :

Coordinates with involved systems of care

Provides follow-up with patients and their families, if appropriate
