Position : SSI - LPN LTC
Location : Brooklyn, NY
Pay Rate : $34 / hr
Schedule : 3pm-11pm. Able to work every other weekend / which includes full time work
Responsibilities :
- Performs tasks and responsibilities within the frame work of case finding, provision of supportive and restorative care under the direction of a Registered Professional Nurse.
- Performs nursing skills and provides nursing care to an assigned group of residents in accordance with the policy of the facility.
- Augments the quality of life of the residents.
- The LPN will function as a member of the nursing team and the interdisciplinary team.
Provides direct nursing care to assigned residents with complex needs as related to :
Hygiene and physical comfortMedical treatmentNutritional needsAmbulation and exerciseElimination Maintenance of residents' safetyMaintenance of residents' SafetyDirects the care of the residents performed by Certified Nursing Assistants.Administers medication to assigned residents with exception of the following :
Blood and blood productsHanging the first IV solution containing medicationsAdministering IV (push) medicationAdministering IV medications via central Lines and Maintenance.Administers IV therapy with exception mentioned in (3) above :
LPN's certified in IV therapy by Schulman and Schachne may perform :Venipuncture for the purpose of placing a heparin lock or administrating IV fluidsSet up and assist in starting IV fluids.Hanging fluids and monitoring fluid rate as presented by physician order.Replacing bags of IV fluidsChanging intravenous dressing, tubing on peripheral lines.Discontinue IV fluids.Administers treatment according to facility policy and procedure.Prepares residents for diagnostic tests, operative procedures, medical regimen examinations and transportation.Admits residents according to facility and unit policies and procedures and assists them in adjusting to the environment.Participates in the Person Centered Care planning and coordinating of the nursing care in an assigned unit and implements plan.Assists and / or contributes to the development and updating of the Person Centered Care Plan to reflect the residents' needs.Assists RN with health teaching for residents and families as needed.Establishes positive relationship with residents and families in accordance with Residents' Bill of Rights.Participates in unit / team ICP meetings and PI teams.Observes nutritional need of residents'. Actively participates at mealtime by assigning staff, feeding residents, monitoring intake of residents consumption.Assist in post-mortem care of deceased residents' as needed.Skills :
Demonstrates competence, and experience in nursing skill within the scope of practice.Has the ability to establish positive team moral and good interpersonal relationship with residents, families and co-workers.1+ years relevant experienceNeed to have NH experienceRequirements :
Completion of LPN programLicensed and currently registered as a Practical Nurse in New York State.BLS (American Heart Assoc.) or NY State CPR cert PreferredCurrently registered as a Practical Nurse in RN Venipuncture of supportive and restorative care under the direction of a Registered Professional Nurse.
