\n Shift : Days, 8 : 00 AM – 5 : 00 PM (No weekends, on-call, or overtime)

\n Schedule Type : Full-time, Monday–Friday

\n Travel : Local travel within the Colorado Springs area

\n\n\n Overview \n\n

We are seeking a skilled and compassionate Registered Nurse (RN) to join our hospice care team. The RN Care Manager plays a key role in assessing, planning, coordinating, and delivering high-quality care to patients in the community setting. This role is ideal for nurses who thrive in a collaborative environment focused on patient advocacy, clinical excellence, and continuity of care.

\n\n\n Primary Responsibilities \n\n \n\t

Perform comprehensive assessments of patients with complex or high-risk needs.

\n\t\n\t

Develop and implement individualized care plans tailored to each patient’s goals and medical conditions.

\n\t\n\t

Collaborate with physicians, social workers, discharge planners, and other care providers to ensure a coordinated approach to patient care.

\n\t\n\t

Facilitate smooth care transitions between settings and providers, ensuring continuity and quality of care.

\n\t\n\t

Evaluate and adjust care plans to maintain cost-effective and appropriate levels of care.

\n\t\n\t

Identify and address social determinants of health such as housing, transportation, and community support.

\n\t\n\t

Promote wellness and illness prevention strategies.

\n\t\n\t

Maintain detailed and accurate records of all clinical, functional, and fiscal activities.

\n\t\n\t

Serve as a patient advocate while upholding privacy and confidentiality standards.

\n\t\n

\n\n\n Qualifications \n\n \n\t

Associate’s or Bachelor’s Degree in Nursing (BSN preferred).

\n\t\n\t

Active and unrestricted Colorado RN License or Compact Multi-State License .

\n\t\n\t

Prior experience in Hospice Care or Home Health setting preferred.

\n\t\n\t

HCHB (HomeCare HomeBase) experience strongly preferred.

\n\t\n\t

Reliable transportation and valid auto insurance required.

\n\t\n\t

Strong critical-thinking, communication, and decision-making skills.

\n\t\n\t

Case Management Certification (CCM or equivalent) preferred.

\n\t\n

IDR Job ID #468479. Pay package is based on 8 hour shifts and 40 hours per week (subject to confirmation) with tax-free stipend amount to be determined.
