New York Licensed RN Case Manager

A company is looking for a Field RN Case Manager.

Key Responsibilities

Manage and coordinate care for patients, ensuring adherence to care plans

Conduct health-related assessments and facilitate the care planning process

Provide support and resources for individuals with chronic conditions and social determinants of health needs

Required Qualifications

Associate's of Science in Nursing (ASN) degree

Active and unrestricted Registered Nurse (RN) licensure in New York

Minimum of 3+ years of nursing experience

At least 2+ years of case management or home healthcare coordination experience

Experience with Medicare and / or Medicaid members is preferred
