Job descriptionPsychoeducation for patients and their support systems. Medication adherence counseling and disease self-management counseling. Motivational Interviewing to develop behavioral strategies aimed at symptom reduction. Cognitive behavioral intervention aimed at modifying negative thinking and promoting self-efficacy. Trauma informed care approach. Develop and monitor person-centered treatment plans that utilize SMART goals. Self-Care Plan development and skills training to facilitate disease self-management, improved coping, distress tolerance, stress reduction, and relaxation; and Substance use disorder evaluation, identification of maladaptive coping strategies, and development of harm reduction strategies. Interview potential patients to assess needs for mental health and / or substance abuse services or referral elsewhere; counsels or provides psychotherapeutic and rehabilitation services for individuals, couples, or members of a family to assist them in achieving productive social adjustments, reducing the impact of disabling disorders, and facilitating their achievement of living, vocational, recreational, and interpersonal goals. Plan and administer therapeutic treatment, behavior modification, and stress management therapy to assist patients in developing / displaying appropriate behaviors. Conduct individual, couples, family, and group therapy. Document clinical activity presenting diagnostic and interview findings, clinical evaluations, and recommendations; as a member of an interdisciplinary team, reviews diagnosis and treatment plans of clinic staff; and may act as a case manager by coordinating services for clients to assure continuity of care and to assure that clients receive appropriate types of service. Confer with and advise patients’ relatives to secure their understanding of and cooperation in treatment and rehabilitation programs. Educate patients and their families regarding their diagnosis, prognosis, treatment, and rehabilitation process. Manage the care of patients by assessing and referring them to public and private providers and monitoring their progress. Provide crisis counseling for individuals experiencing acute episodes; evaluate patients and arrange for voluntary or involuntary admission to facilities for observation and treatment. Consult external community providers concerning treatment plans and for coordination of care, as required. Assist patients to obtain stable housing, food security, physical health care, benefits, transportation, and other enabling services to enhance social determinants of health and improve overall health outcomes. Maintain regular contact with other agencies and professionals regarding patient progress in the therapeutic treatment milieu through phone calls, letters, and / or monthly reporting, as well as documenting within the EHR System (EPIC). Maintain electronic case records that are in accordance with HIPAA guidelines and other federal and state regulations. Culture built on patient care, teamwork, and community impact. Competency in assessment, diagnosis, prognosis, counseling, and psychotherapeutic treatment of AODA, marriage, family, and child relationship dysfunctions; methods of developing and implementing a treatment plan; current trends in the field of mental health; laws and regulations applicable to mental health; and basic math. Knowledge of the spectrum of mental illness, symptomatology, psychotropic medications, and the problems presented in adjusting to life cycle changes. Ability to perform effective counseling and psychotherapy with individuals, couples, families, and groups; develop and maintain cooperative relationships with patients’ families, members of other professional disciplines, social agencies, and co-workers; relate professionally with persons with serious mental disorders; identify and utilize community resources; advise and train other mental health staff; and prepare and present clear and concise oral and written reports. Competency in providing cognitive and behavioral interventions to individuals, families, couples, and groups with behavioral health / substance abuse problems. Excellent interpersonal skills and ability to work collaboratively with patients, substance use disorder counselors and other behavioral health clinicians. Applicants must possess WI state licensure in their profession at the time of application or ability to obtain within 6 months (LCSW / LPC / LMFT). The selected candidate for positions in this class are required to undergo Noble’s Provider Credentialing and Privileging which includes extensive background investigation. Per Federal Grant requirements, the selected candidate must not be listed on any debarment or exclusion list.
