The hospital is looking for Physician - Emergency Medicine in Eau Claire, WI

JOB DESCRIPTION :

SUBMISSION REQUIREMENTS :

ABEM / AOBEM Board Certificate or Board eligibility letter
Must have a clean file / background no malpractice
Procedure logs (last 2 years)
Updated CV, drivers license, color photo, state medical license (including licensure held in other states), CSR, DEA, education certificates, 5yr malpractice history, need narrative & legal documentation if provider has claims, ECFMG if applicable, ABEM / AOBEM Board Certificate, Board eligibility letter, TB / PPD / Quantiferon / Chest x-ray (within 12 months), COVID, and FLU (seasonal).

AS A REMINDER :

All candidates must have met the minimum shift availability listed on the job.

CV must have all dates in MM / YY to MM / YY format

Work History must be in Sequential order with most recent on top

Any Gaps over 30 days must be explained on the CV

EPA (Please use the attached EPA. Its also in Trio under your Name & then click on Help)

All Licenses, Boards and Certs with expiration dates must be listed on CV

Anyone with malpractice must provide a NPDB

Anyone with legal issues must provide supporting documents

Availability

1st possible date they would be available?

How many shifts would they be able for per month?

What month is their schedule completely open?

Any shift preferences (Day, Night, Blocksetc)?

Any dates in the next 3-6 months they cant work?

Any Holidays in the next 3-6 months they would be open to work.?
