Job Title : LPN

Description :
The LPN must demonstrate the knowledge and skills necessary to provide care appropriate to the age of the patients served on their assigned unit.
The individual must demonstrate knowledge of the principles of growth and development across the lifespan.
They must possess the ability to assess data reflecting the patient's status and interpret the appropriate information needed to identify each patient's requirements relative to age-specific needs.
The LPN will provide direct and indirect skilled nursing care for assigned residents.
They will follow defined policies and procedures under the supervision of a Registered Nurse.
The LPN will maintain a collaborative relationship with other nursing staff, physicians, and ancillary staff.
They will maintain an appropriate level of knowledge and clinical expertise through continuing education and in-service programs.
The LPN shall possess the ability to provide care to geriatric residents.
The LPN will collect data relevant to residents' physical and psychosocial conditions on an ongoing basis and communicate findings to the RN and other members of the Interdisciplinary Team.
They will contribute to the creation of a developmentally appropriate and culturally relevant plan of care reflecting nursing diagnoses and interventions to achieve desired resident outcomes.
The LPN will involve the resident / family and healthcare team in the care process while promoting a healthy, safe environment.
They will recommend revisions to the plan of care after evaluating resident outcomes and indicating the need for referrals to ensure all aspects of the resident's care have been addressed.
The LPN will collaborate with residents / families and members of the Interdisciplinary Team within the scope of nursing practice to assure customer satisfaction.
They will maintain current knowledge and competency in Nursing Practice.
The LPN will participate in the mentoring / orientation of staff.
They will consider factors related to safety, cost, and impact when delivering care.
Under the direction of the RN, the LPN will provide leadership in the professional practice setting.
They will demonstrate knowledge and competency in their role.
Occupational exposure to bloodborne pathogens is reasonably anticipated.
Required tasks may involve potential contact with blood, body fluids, or tissues.
